'All in one' care tracker wins HHS innovation competition

Axial Exchange’s Care Transitions Suite, which integrates multiple sourcing of patient information from pharmacies, hospitals, EMS, physicians and health plans, won a first prize award in Health and Human Services’ (HHS) Office of the National Coordinator for Health IT (ONC) innovation competition. The Care Transition Suite provides access portals for all these entities, plus a patient-facing portal to help with post-discharge care, all accessed on a smartphone or tablet. For being judged the top technology, Axial won $25,000 and the chance to demonstrate at the CMS QualityNet Conference. Directing judging was Health 2.0‘s Jean-Luc Neptune [TA 21 Aug conference report], but he noted for FierceMobileHealthcare that the service was still at early beta stage, the company was still quite small and they were still working out integration issues. Integrating multiple sources and multiple portals for the information is certainly the way to go…yet with all the pressures of EHRs, data breaches, CMS payment reduction for 30-day same-cause readmissions and other technologies banging on the HIT door, can Axial’s ‘well done, elegant’ service find a profitable niche?
