Help in ADOPTing technology for older adults

The Center for Technology and Aging (CTA) this week launched its new ADOPT Toolkit to assist healthcare organizations of many types–whether hospitals, providers, home health agencies, payers and aging social services organizations–to better use the right technologies for the right people at the right time. The ADOPT Toolkit has tools for four areas: Remote Patient Monitoring, Medication Optimization, Care Transitions, and Mobile Health. These are then organized into ‘workstreams’ that lead adopters through the complex steps of a successful deployment. Most of the materials are now available for download, with some to come later this year (registration required.) The toolkit was funded by the SCAN Foundation, which supports health for older adults to preserve independence, and The Commonwealth Fund.
