Technologies key in preventing unplanned hospital readmissions: report

A report compiled by the California-based Center for Technology and Aging highlights the role of technology in reducing preventable readmissions (18% of patients are readmitted within 30 days, an estimated 76% of them are preventable.)  Highlighted are four post-acute care transition (PACT) technology areas:  medication adherence, medication reconciliation (adverse drug events), remote patient monitoring and health information and communication (including PHRs and remote training).  Release.
