SeniorBridge, a medical home care management company primarily operating in the Eastern US, and Ohio-based VRI are rolling out a joint program, ‘SeniorBridge Telecare Management’. Blood pressure and weight scales will be monitored through devices connected to a home phone hub. What is different is a ‘triage’ style model–out of norm or no reports send alerts to a VRI call center representative, who will first call the older person to obtain information, then call the SeniorBridge care manager. SeniorBridge care staff use monitoring information to review with the patient as part of their assessment.
